if you look at all the addons mar posted, im sure you can figure it out for yourself 
just go to curse or ui.worldofwar - and search for the name.
My guess is that its clean minimap perhaps? as the name would suggest.. |

Minimap looks like 'SquareMinimap' which is not ann Addon in itself, but some files that need to be extracted into WoW's directory. (Though since 1.10 AddOns might reproduce the layout done by SquareMinimap).
The 'Improved Chat' looks a lot like some plain new chat tabs, some dragging around and enabling/disabling one *hell* of a lot of options. |

I'm a bit curious on how you were able to hide the Chat scrolling-buttons? I looked for a suiting addon on Curse-gaming and found this addon but didn't get it to work.
Also tried looking for an appropriate addon in the list you posted earlier in the thread but had no luck there either.

I cant find same addons from curse-gaming.
Some i do found, and some are diffrent version and so on...
MAR can you just do .rar file from yours and share it?
My guild is in c'thun now... taking advices from loot ftw's videos.
